Amber is a 35-year-old Caucasian female client who resides in a rural community within Newfoundland and Labrador. She has a family history of cardiovascular disease (CVD). Amber was recently diagnosed with hypertension weeks ago during her annual physical exam. Amber’s healthcare provider was hesitant to start her on medication at this time, with a preference for her to focus on modifiable risk factors first. During this visit, Amber reluctantly agrees to sign up for a series of chronic disease self-management sessions with the community health nurse in the neighbouring area. Today is Amber’s first session with community health nurse Sarah.
