Danielle is a first-year nursing student who is currently midway through completing her first clinical rotation in Long Term Care with her clinical instructor Kate. Kate and Danielle are heading in to provide afternoon incontinence care for Mrs. Brown. Mrs. Brown is an 85-year-old elderly Caucasian female who was diagnosed with severe vascular dementia after suffering a stroke 3 years ago. Mrs. Brown displays a flat affect and is unable to communicate verbally due to expressive aphasia. Danielle will complete the patient assessment and assess newly developed skin breakdown on the patient’s coccyx area.
