Christian is a 2nd year nursing student and currently has clinical placement on a long-term care floor. Christian arrived on the floor for his morning shift at 0700, Christian is assigned to Mr. Johnson, a 69-year-old male who has been admitted for a urinary tract infection. His past medical history includes hypertension, a history of smoking, bending prostatic hyperplasia (BPH), and mild dementia. Upon receiving report from his co-assigned nurse, Christian learns that Mr. Johnson has been exhibiting symptoms of delirium. Mr. Johnson has been making inappropriate comments about the staff on the floor. Christian goes into his room to start his morning assessment.
